NOTE ON DATES: IMPORTANT: For consistency, the Date is given as the date the mission TOOK OFF since the precise time of a loss is not always certain. Take Off date is unambigous and fixed in the official records, but obviously in those cases where the incident occurred before midnight UK time, then the Take Off Date will be the same as the Incident Date. Of course, most Bomber Command missions flew through midnight, therefore a Luftwaffe claim against a plane - or a locally generated crash report - may record the incident as occurring on the day following our Take Off Date. Bear this in mind when cross-referencing to our Luftwaffe Victories by Name/Date Database and other Luftwaffe sources. In some cases other sources may quote the date following our date, using locally generated reports as their source. To add to the potential for confusion, remember to take into account a Luftwaffe recorded date will be in local time, 1 hour ahead of UK time. When we discover a validated Incident Date we change our record if necessary.

Took off at about 1505 hours on board an Anson I (N5130) piloted by Flight Sergeant M O Samuels on a cross country navigation exercise. The plane observed about 30 minutes later at 5000 feet heading north, then turning to port and entering a near vertical dive as part of a wing broke away. The wreckage fell near Marl Farm, Llandudno Junction, Caernarvonshire, and all five crew died. It is thought that the wing failed after an aileron separated and led to loss of control. Crew: Pilot Flt Sgt Melville O Samuels (Age 23), W/Op Flt Sgt Thomas M Clothier (Age 23), P/O Charles HM Folkard (Age 21), Nav Flt Lt Jan M Radecki Age 30), W/Op Sgt Sylvester K Yates (Age 21).

The operation was continues but twenty minutes later the bomber was shot down by a German night-fighter and it crashed into the sea, near the Island of Borkum at 23.50 hours. The two survivors were adrift for eight days in dinghy before being rescued by the German Kreigsmarine.. Location : North Sea / Germany / Denmark.PoW. Died 12.04.2004 Devon, U.K.

However, there is no evidence in the definitive Night Fighter/ Flak publications nor in the records of Polish Air Force losses that support this claim. Therefore the reason for the loss of the a/c is unknown.

The bodies of the Pilot, Navigator and an Air Gunner were washed up on the Lincolnshire coast, but there was no trace of the other three members of the crew . Body not recovered until early May 1942.
