REASON FOR LOSS:

Taking off at 12:35 hrs on anti-submarine patrol.

The operation had been hastily prepared, the navigator lacked experience and confidence, and the weather was bad. The weather and breakdowns in navigation and radio procedures led to the aircraft becoming lost.

Repeated attempts to pass navigational assistance were unsuccessful and early in the evening, the last radio transmission was an SOS message that the aircraft had run out of fuel and was going down.

Searches were mounted and continued until dawn the following day but the crew were never found.

The subsequent court of inquiry led to substantive changes in the management of navigational aids and mission preparation.
